"Space Marine" Servo Skull Guide: Belly of the Beast & Titans of Graia (Chapters 3 & 4)
This guide covers the locations of all of the collectible Servo Skulls which are located in chapter 3 "Belly of the Beast" and 4 "Titans of Graia" in Warhammer 40,000: Space Marine. These chapters are combined due to the limited number of skulls between them.
All of the 48 servo skulls in the game are located throughout the various levels, typically in out of the way locations, with each providing some form of back story and lore to the events taking place. This come in the form of short recordings left by other people on the forge world. Collecting all of these, besides providing interesting information, contributes towards unlocking special achievements.
Chapter 3: "Belly of the Beast" Servo Skulls

Chapter 3 is a decent sized mission game-play wise, however for whatever reason, there are just not that many skulls to find here. Both are evenly spread out through your travels inside the planetary gun fortress as you attempt to take control of it away from the Orks.

The first servo skull is found in the very beginning of chapter 3 during the "Enter the Gun Fortress" objective. This will be when you first enter the gun fort and encounter the ammo loading belt, bringing shells to the upper floors. When looking at the belt, turn to your right to find a room partially obscured by rubble. Inside this room and to the right again, behind some crates, is where the servo skull will be.
"Transmechanic Second Class Petronius, entering duty log 31127. Communications console functioning: normal. Issues of note: one. I detected a signal that should not have been there, but Transmechanic First Class Aleric refused to believe me. Yes, I've made mistakes before. That pulsar I thought was a Tau warship. Missing the signal from the Fabricator General's transport.
But I am not wrong every time! There's something approaching from the far side of the sun. I know it."

Later on in the level during the "Sabotage an Artillery Shell" objective, when you've first wrapped things up on the main floor you first entered, you will take an elevator ride to another level with catwalks in the back. Here you will find a large numbers of crates everywhere with a more open view behind everything else, into another larger room with many pillars. This room in the back is actually the ammunition storage room for the facility, with the columns being containers for the slugs loaded by the gun fortress. Look at the far end of the room with your back to the elevator and you should see the glowing red eyes of the servo skull next to the railing.
Lt. Mira: "-- not enough cover -- Get down, get down!"
Captain: <agonizing scream>
Mira: "Hold on, Captain! Get me a medi-pack!"

Chapter 4: "Titans of Graia" Servo Skulls

Because most of this chapter involves riding the Ork Battering Ram and the entire combat sequence that takes place within it, there is a good reason for there being so few skulls here. Most of the regular level type terrain you'll find is only in the first few minutes of the chapter, limiting the places where they realistically could be hidden.

The one and only servo skull that there is to find in chapter 4, "Titans of Graia", is located in a small alcove which can be found right before crossing the bridge onto the Ork Battering Ram train. If you do a 180 from this point, you'll see said alcove and a turn to the left which leads you a little bit further down a hall overlooking the main area, with the skull sitting right at the end.
Dominus: "-- has a deep wound to the arm. Signs of blood poisoning."
Magos Cassia: "That's from an Ork choppa. We can't stop him from going septic. Make him comfortable and move on."
Dominus: "You've seen these kinds of wounds before?"
Cassia: "Unfortunately, yes -- in my first Ork invasion, which I hoped would be my last. Yet, here I am again. Right, who's next --"
